Hey folks -- I am Looking for a workshop outline on sexual health issues
in clinical practice. We will be doing a training for medical and
nursing students on working with clients in a community clinic setting.
The goal of the training will be two fold: first to increase their
comfort in talking with clients about sexual health and second to
increase their general knowledge base on sexual health issues. Anyone
have ideas of where to look?

Fellow HEDIRS and drug educators,

As a college prof I teach a great deal in the area of drugs and alcohol. I'm also an EMT/firefighter in a rural fire district. The two worlds are often far apart but sometimes they collide. The collisions have made me a better educator and a better rescuer but the process is sometimes difficult and frustrating. I wish I could take my backrow of side-talkers and eye rollers out of my Drugs and Health class with me on a few late night calls.

This afternoon I was writing to an old buddy, a fellow health education prof., and airing my frustrations (it's cathartic) and when I finished I read what I had written and I thought I might share part of the note with you. I've not changed a word so bear with me. It's guy's talk, not scholarly writing. It may offend but it may be of value. If you find it valuable, "de nada."

Phew! speaking of ugly scenes we had a bad code last night around 12 am. Call came in as "possible alcohol poisoning" and then while en route dispatch says, "code 99, CPR in progress."

Seriously, bro, it hits home. This guy was just the age of most of my students--the ones that we try to teach in our drug and alcohol classes. They just can't believe that booze is anything but a fun time. They can't believe that it can kill you. This guy was partying with his friends drinking beer and taking straight shots of "Mexican white lightening".

He leaves the party around 9 and dies on the front seat of his pick-up in the driveway. Around 11:30 somebody notices his truck still parked out front. They yard him out and into the living room all covered in puke and diarrhea and start CPR so when we arrive we have to continue the resuscitation and run a full code (we have to, it's our protocol) even though he's had 3 hours of "unaccounted for time" and because he was found under hypothermia conditions. Who knows how long he was down. Anyway we worked him for 30 minutes in the house and all the way to the hospital---total flatline. Never anything close to a shockable rhythm.

I wish I could accurately describe the grimness of these scenes: drunk girlfriends crying and wailing, buddies yelling at each other, at us, at their dead buddy, the cops questioning them, and five firemen who've just been jolted out of their sleep are trying to pull off the impossible on the first dead person these kids have ever seen or known.

We're siphoning chunks out of his airway, trying to tube him, breaking his ribs with compressions, and trying to get a shock or two on board. All the while he's spouting beer like a whale and crapping out the other end. The dog is barking and radios are squawking and we finally tube him, stick him, and load him and go. It's dead quiet in the rig and we talk about the lack of traffic and if anybody got his name. We drop him in the ER, give our report and then head for home and I wonder what his mom's going to think when the deputy calls her in the next hour or so.

It's one thing to run a code on a 80 year old man with congestive heart failure but a 20 something who was just out for a party....what a freakin' waste! I told my college kids today, "There's no good way to die but some ways are more preferable than others." Going to God drunk and stupid and covered in puke and shit is no way for anybody to leave this mortal coil.

Tell this to your back row of students who snicker and roll their eyes when you talk about the risks of alcohol.

Patty, if you check Medline you'll get some good articles and from an initial
search you can find other related articles. The Internet address is:
http://www.igm.nlm.nih.gov That's the address for Grateful Med that I'm
used to using (PubMed etc. also available). A simple search of: community
health aides and delivery of health care and United States with a limit of
1990-present will do the trick. If you check PubMed you should be able to get
book cites. This was just a "quick and dirty" search. Hope it helps. Gayle
