I have finally compiled the responses sent to me regarding a cancer quality of life instrument. Please delete if not interested.



The most valid and reliable measure is the MOS scale, of which there are several versions. Dr. John Ware is the author and he has spent years on his instrument. He and his colleagues wrote a book about the Medical Outcomes Survey (I think it's called). I've used his questionnaires in my research.

If I were you I would check with the American Cancer Society National Office
in Atlanta. They are undertaking a huge QOL study in the US.
Take a look at the European Organization for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaire. The instrument is a modular system for evaluating the quality of life of cancer patients participating in international clinical trials. It has 36 items and has documented statistics for both reliability and validity. You can find a copy of it in the book "Measuring Health" by McDowell and Newell ISBN # 0-19-510371-8 The library of congress number is RZ408.5.M38 (I think) if you want to see if your library has a copy. It is printed by Oxford University Press in 1966. I hope this helps.

I am currently using the FACT questionnaire (Functional Assessment of Cancer Therapy) by Yellen, Cella, Webster et al. There are several versions that are specific to the types of cancer as well as the cancer symptoms.

There is at least one good measure... see Ferrell BR,
K Hassey-Dow, and M. Grant (1995) Quality of Life REsearch 4, pp.
523-531 "Measurement of the quality of life in cancer survivors".

One of the most widely used, and the one we have used the most here, is the Functional Assessment of Cancer Therapy (FACT). There is a core instrument of about 28 items, measuring quality of life in 4 major domains, and there are add-ons specific to a variety of cancer sites (e.g., breast, lung, head and neck, etc). I would think this would be relevant to survivorship. I know the instrument is used as a followup measure after people have completed the active phase of treatment. Other possible instruments include the Functional Living Index - Cancer (FLIC), the Cancer Adjustment Survey (CAS, Friedman, 1988), and the Mental Adjustment to Cancer & Mini-Mental Adjustment to Cancer scales (MAC & MiniMAC, Watson). Others not specific to cancer include the Psychosocial Adjustment to Illness Scale (PAIS), and the Sickness IMpact Profile (SIP). Beyond that, many studies utilize standard measures of psyc!
hological functioning, like the Beck Depression Inventory (BDI), Hospital Anxiety and Depression Scale (HADS), or the Center for Epidemilogical Studies - Depression scale (CESD).

There is a Caregiver Quality of Life Index-Cancer Scale (CQOLC) developed by
Weitzner et al., 1999, published in the Quality of Life Research Journal.
