Carl,
I would suggest that you contact the American Association of Diabetes Educators
(I'm pretty sure that is the name of the organization). They might have the
resources you are looking for.
Thanks!

Dear colleagues,
I need some advise for one of my faculty members. This faculty member is 64
years old and has multiple health problems including diabetes, high cholesterol
and hypertension. Most recently he can not wear shoes due to circulation issues
related to the diabetes and psoriatic
arthritis. Since he knew my background is in health education, he
asked me if I know of some medical center in the United States that has
expertise related to the diabetes and the circulation problems in his legs. I
think this faculty member is willing to travel to a center that may be able to
help him.
Can anyone recommend medical centers in the U.S. that are on the cutting edge
that could help this person? He is willing to travel to the medical center.
Thanks.
Carl
