I would love to weigh in on this issue. There are many of us who have worked virtually all of our careers in the areas of social determinants.  I will stick to my current position. 

 

Around 2000-2001, the environmental health services in Multnomah County (Portland Metro area) conducted a community needs assessment using the PACE-EH model.  The assessment focused on the areas of greatest health disparities as identified by several things such as asthma rates, diesel particulate exposure, brown-fields and EPA cleanup sites, etc..  Surprise, surprise these maps overlay well with the maps of minority and refugee populations, crime rates, poverty, unemployment, concentration of low income housing (substandard in many cases), uninsured rates.

 

The short story is that the community identified, mold, lead, indoor air quality and trash as the primary environmental health issues that they confronted but the number one issue that was perceived was "feeling ignored."  The typical response was "you all at the county, come in here, do your assessment or health program and six months later you leave without giving us the resources to solve our issues."  Out of that work, our environmental health services creates a new initiative area called Community Outreach and Education, with the charge to work on program and policies in a long-term systemic way.  The first big funding department received a HUD grant to work with low income families who had children with asthma.  I was hired to manage this service group about this time.  Our approach to the HUD grant was to use a team approach and work with families over 6 months.  An environmental health nurse provides help for families to understand asthma control.

 

and medication (customizing and tailoring educational materials) and the nurse helps the families understand how to use the health care system.  An environmental health specialist assesses the house for asthma risk factors (triggers) such as pests, mold, toxic chemicals, tobacco smoke etc..  and community health workers work with the families to help control their environments (clean mold, reduce pests, eliminate bleach use, control smoking).  Okay all of this sounds like typical health education...  But the program does not stop there...

 

In our work with families we identified serious problems in the housing stock --basically, landlords intentionally or unintentionally deferring maintenance etc, so we started working to change policies related to building code and code enforcement.  Collaborating with the City Bureau of Housing and strategically using media to raise awareness of substandard housing, in the last 2 years we have created a huge pressure for change.  And in the last 6 months we have been leading (with the city) a workgroup comprised of health advocates, housing advocates, public agencies and landlord and property management associations.  The workgroup meets next week to begin the process of finalizing recommendations that will be made to city council in Portland that will increase the quality of affordable rental housing in the city.  We also were part of an effort in our other big city in the County, Gresham, who just past a brand new rental housing inspection system.

 

Another component is that we convene a group of community stakeholders to strengthen long term strategic planning,  In the process we are developing new partnerships, that are not typically considered health education.  For example one the recommendations coming out of the workgroup is to develop a strong alternative dispute resolution model that includes culturally driven conflict resolution training, community-building, and providing community mediation services, as we recognize that developing a strong sense of community is another facet of health.  Another partnership that is in its infancy is with a group that is doing economic development with low income and refugee community members and that agency (MercyCorps) is very intrigued by the connection between economic development and health status.  This link between jobs, justice and health is huge.

 

The goal is not only to reduce asthma through health education but also go "upstream into social derminents" to improve housing stock, give underrepresented commuities voice and skills to self organize and adress issues, build community, and link health with justice issues. So if I were to summarize, our "social determinant-based program" is about 10% direct client services (asthma, lead prevention, environmental health education) 30% poilciy development, 30% partnership development, 20% framing the social and environmental justice issues around health equity, and 10% paper pushing.  Fianally, its eight years later and we are still in the community that is ignored.
