Graduate Assistantship Forms Below...to be completely ONLY after admission to the program
Anybody interested in applying for a graduate assistantship with the health education division must complete the following forms as well:
Teaching Assistantship Application: On-Line Version  
Teaching Assistantship Letter of Recommendation Forms: On-Line Version  
Research Assistantship Application: Word Version  
NOTE:  All International Students (exception of Canada and Great Britain) are required to submit a brief (5 minute) video of them speaking on their background and why they would like to get a degree in health education.  This should be submitted in a digital format (i.e., wmv, mpg) and can be sent as an attachment in an email or on a CD.  This must be received prior to April 1.
