MPH Application
All application processes can now be
completed on-line. You must complete the Graduate School
Application (linked below) and submit the application fee of $45 (make
payable to SIU and mail it to the department in care of the Graduate
Director).
In addition to
the Graduate School Application, you must complete the following:
- Additional Health Education
Requirements
- Minimum of 3 references (using
required form below)
- Official Transcripts from all
universities/colleges listed on your application form
International Students Must Also
Submit:
- Copy of their passport photo

TOEFL Scores
- Financial Disclosure
- International Students who, upon
being admitted into the MPH program and upon arrival in the U.S. who
decide to participate in a U.S. Medical/Dental Residency before
completing their MPH will be dropped from the program.

Note that application due dates are as
follows:
- For entrance in the summer/fall
semesters, the deadline is the THIRD Friday of January
- For entrance in the spring
semester, the deadline is September 1
In addition, it should be noted that
applicants who miss these due dates may be reviewed under certain
conditions. Contact the Graduate Director for more information.

NOTE: STARTING FOR THOSE ADMITTED IN
FALL, 2008, a requirement for graduation is that ALL MPH students must
register for the CHES Exam. More information will be forthcoming.
Note:
- The MPH DOES NOT require a GRE or
MAT test
- Individuals interested in
Assistantships must FIRST be admitted into the MPH program. Please
